1.1 Describe the duties and responsibilities of own work role

As a health and social care practitioner, my primary responsibility is to provide high-quality care and support to individuals who are in need. This involves working with people of all ages, including children, adults, and older adults, and providing a range of services, including physical care, emotional support, and practical assistance.

In my role, I am responsible for carrying out assessments to determine the needs and preferences of the individuals I work with and developing care plans that outline the support they will receive. This includes coordinating with other members of the care team, such as doctors, nurses, and therapists, to ensure that all of the individual’s needs are being met.

I am also responsible for monitoring the progress of the individuals I work with and making any necessary adjustments to their care plans based on their changing needs.

In addition to providing direct care, I am also responsible for keeping accurate and up-to-date records of the care I provide. This includes documenting observations, interventions, and outcomes, as well as any communication with other members of the care team. As a health and social care practitioner, I am also responsible for maintaining my own knowledge and skills and staying up-to-date with developments in the field by participating in ongoing professional development opportunities, such as training courses or workshops, and maintaining my professional certification or licensure as required by my employer or regulatory body.

My duties and responsibilities as a health and social care practitioner are centred on providing high-quality, person-centred care to the individuals I work with and helping them to achieve their goals and live as independently as possible.
